Services

The prices set out in this schedule are effective from the date shown on the front cover and are subject to change without notice. All of the items marked (with an asterisk or shading) will be charged as a minimum charge and make up the cost of our direct disposition/transfer services. This list does not include prices for items which we are asked to purchase for the purchaser, such as newspaper notices, cemetery or crematorium charges or other items and services. These items will be shown as disbursements on our final account to the purchaser. Our funeral directors will advise purchasers at the time of arrangements are made if any legal or other requirements mean additional supplies or services must be or should be purchased.

 

Staff and Professional

Professional Services

This service includes, but is not limited to, the availability and work done by our funeral director on behalf of the purchaser. This would include the coordinating and directing the services of our staff and agents with those of the hospital, nursing home, cemetery or crematorium in picking up the remains from the place of death and making the necessary arrangements for final disposition. It also includes the services of our support staff in preparing contracts, correspondence and invoices and in placing notices, booking staff, vehicles and other support function.

Documentation

Completion and filing of all documents necessary to carry out the services and supplies requested, including, but not limited to, death registration, burial permit, coroner’s certificate for cremation, documentation necessary to ship the body out of the country. As part of this service, we will provide you with 12 proof of death certificates; such certificates are usually accepted for most estate settlement purposes, but sometimes a provincial Death Certificate will be necessary and is available from the Office of the Registrar General.

Transport remains

Transportation of the remains from one place to another, for example the hospital to our premises, or from our premises to the cemetery or crematorium using a transfer vehicle, for distances not exceeding 25 km. a charge applies for each transportation request. Additional charge for distance (mileage) ($1.95 per kilometer) one way.

Basic Staff

*Coordinating rites and ceremonies: Co-ordination of all rites and ceremonies, before, during and after they have been provided, including services and supplies provided by us, as well as those provided by third party suppliers, such as flowers and death notices. This service includes but is not limited to staffing, cleaning and maintenance of premises and ground, staffing office and telephone coverage.

Staff for Ceremony (onsite or offsite)

This service includes but is not limited to, preparations for funeral service or memorial service or graveside service, staff to direct and assist with the funeral service or memorial service or graveside service and includes a period of time up to four hours in length. This service would include receiving, arranging and local delivery of flowers, coordinating donations, etc. at the funeral home or offsite.

Staff for Ceremony with 1hr Visitation Prior (onsite or offsite)

This service includes but is not limited to, preparations for funeral service or memorial service, staff to direct and assist with the funeral service or memorial service and includes a period of time up to five hours in length. This service would include receiving, arranging and local delivery of flowers, coordinating donations, etc. at the funeral home or offsite.

Staff for Graveside Ceremony

This service includes but is not limited to, preparation for graveside service at the cemetery or burial location, staff to direct and assist with burial and all facets at leading up to and following the interment. Additional staffing for offsite ceremony available.

Staff for Initial Period of Visiting (Approx: 2 1⁄2 Hours) onsite or offsite)

This service includes but is not limited to, preparing for visitation, staffing during visitation, receiving and arranging floral tributes, coordinating donations and other duties associated with rites and services during visitation. Additional staffing for visitation offsite available.

Each Subsequent Period of Visiting (onsite or offsite)

This service includes all the services itemized in the description of 2 1⁄2 hours of visiting and is charged on a per diem basis.

Embalming of remains (requires basic preparation of remains and shelter)

Embalming is not a requirement of the law. Some funeral arrangements, such as a funeral with viewing or the holding of a body for an excessive period of time between death and final disposition, may require that embalming or disinfection take place. The funeral home staff will use their professional judgment to determine on a case by case basis whether embalming is recommended.

Other Body Preparation (requires Facilities for preparation/embalming/shelter)

These services may include, when necessary, cosmetology, dressing of body, hair dressing, placing the body in the casket or container, special care of autopsied remains, sanitation and exterior disinfection of the body when no embalming is authorized.

Pallbearers or Extra Transfer staff

When there is no procession to cemetery or crematorium and or no pallbearers to assist following the ceremony. Extra staffing for transfer and assistance when the remains is in excess of 400 lbs.

Staff for Burial of Ashes

Funeral director to arrange and attend to burial of ashes or storage at funeral home of remains when not immediately following a ceremony at the funeral.

Facilities

Basic Facilities

This is a minimum charge made by us in all cases for the use of our facilities. Use of our general and common areas, parking lot, public washrooms, cloakrooms, heating, lighting and maintenance. This charge applies whenever we provide services to you.

Facilities for prep/embalm/shelter (any remains that enter site is charged)

Facilities used to prepare, embalm and/or shelter the body; this charge applies whenever the body is in our facility for any purpose. Includes use of all necessary equipment for professional or sanitary care of the remains together with a room for this purpose.

Facilities for Initial 2 1⁄2 (approx) Hour Period of Visitation

This includes the use of our facility for reception of friends and relatives and may include any or all of the following: register desk, casket bier, flowers and donation stands, sound system and religious equipment.

Facilities for each subsequent visitation (2 hours approx.)

This is a per diem charge for the use of our facility for the reception of friends and relatives for each subsequent hours of use after the first period of visiting. Included in this are the items previously listed in the 2 1⁄2 hour period of visiting.

Facilities for ceremony (requires Staff services for ceremony)

This includes the use of our premises, furnishings and equipment the day of the funeral service or the memorial service. This may include any or all of the following: lectern, organ, sound equipment, religious equipment, flower and donation card display stands, casket bier, casket carriage.

Facilities for Ceremony with one hour of visitation prior

This includes the use of our premises, furnishings and equipment the day of the funeral service or the memorial service. This may include any or all of the following: lectern, organ, sound equipment, religious equipment, flower and donation card display stands, casket bier, casket carriage.

Reception facilities “Smith Family Centre”

Use of facilities for a reception following the ceremony or memorial service, which includes the set up of the facilities, the use of the facilities and amenities.

Tribute Equipment

Use of TV/DVD/VCR or projector in funeral home or other facility for display of personal tributes provided by someone other than the funeral home.

Motor Vehicles

The prices shown are for the use of the vehicles on a single occasion, such as use of the funeral coach from the funeral coach from the funeral home to the church and then to the cemetery where the trip does not exceed 25 kilometers from our funeral home. An additional charge (1.95/km) may be made for the use of all vehicles for each kilometer beyond this area.

Utility Vehicle

Utility VehicleThis vehicle is used by our staff in making arrangements, picking up and filing documentation necessary for our services and any other trips that are necessary in conjunction with the services being performed by us. The charge for this vehicle is a flat fee for all funeral services performed by us.

Funeral Coach/Hearse

HearseThe use of this vehicle includes the transportation of the deceased from our facility to the place of the Funeral rite and to the place of final disposition. This vehicle will also be used for the safe delivery of remains to crematoriums, other funeral homes, airports, etc.

Clergy/Lead Vehicle

Clergy/Lead VehicleThe use of this vehicle may include any or all of the following uses: transportation of clergy to funeral home or place of funeral rite, transportation of clergy to place of final disposition, leading of funeral procession.

Service Vehicle

Service VehicleThis vehicle is used by our staff for delivery of floral tributes to cemetery, church or other locations instructed.

Extended Funeral Care

Our services at Smith Funeral Home extend well beyond the funeral. We know you will find there are so many practical matters to take care of during this difficult time. Be assured we will do all we can to assist in the days to come.

As part of our Extended Funeral Care service, I will help with important matters such as completing applications for Canada Pension Benefits, cancellation of Health Card and Social Insurance, Pensions and Insurance Claims and many other practical items that can be confusing and time consuming.

Within a few days I will call you to set an appointment.

It is necessary for you to have the following Documentation in order for us to complete the necessary paper work:

  • Health Card and Social Insurance Card
  • Passport, Nexus Card, Citizenship and Immigration Card
  • Void Cheque for direct deposit of CPP Survivor Benefits
  • Surviving Spouse Social Insurance Number, Marriage Certificate and Birth Certificate
  • Will (Proof of Executor)
  • Insurance Policies
